Anyone with a life-limiting illness in need of Palliative & Hospice Care is eligible to come here.

We need the patient’s doctor or specialist to fill up the hospice referral form and provide the necessary medical reports. Upon submission by fax, our administration will contact the referring hospital ward nurse or family member for follow-up. Click here for more information about admission procedures.
