What Must I Do

 

You will need to get the doctor or specialist to fill up the hospice referral form and provide the necessary medical reports to us.

What Documents Must I Bring

 

Please have the following documents with you for admission:

  • Patient's identity card or passport
  • Your NRIC for registration
  • Patient's medical report, X-rays, and other documents from the hospital

    • What is the Waiting Time for Admission

     

    Upon receiving the hospice referral form by fax, our administration will contact the referring hospital ward nurse or family member for follow-up within 1 working day. Upon assessment and acceptance of the referral, the patient can be admitted the next working day if bed is available. There may be a delay in admission to our in-patient wards if the capacity is full; in which case, the referral will be placed in the waiting list for the next available bed.

    For home and day care referrals, the first visit or contact by the team will be within 3 working days.
