Home Care Support

Home Care is a 24-hour support service which caters to patients who choose to stay at home. We will assist you and your family as you cope with your current condition. Home Care is committed to helping you adjust mentally and emotionally, with your loss of health and independence

Service Features

* A support team of nurses, doctors and counsellors/ social workers
* Regular home visits by a nurse on weekdays between 9am to 5pm
* Visits by the doctor when medical attention is needed
* Training in basic nursing care to the main caregiver to help ensure the patient’s comfort at home
* Loan of medical and nursing equipment whenever possible, such as wheelchair, walking aid, commode or oxygen concentrator
* On call service 24 hours a day, seven days a week
* Provides medical and nursing advice and medical attention
* Pain and symptom control, psychosocial and spiritual support


Referrals:

We require your doctor or consultant to complete the Community Referral Form and to submit it with a medical report.  It can be sent via fax to us at 6235 3512.

When the patient is discharged from the referring hospital, the Home Care nurse will contact the family to arrange a home visit.

Transfer of care

Patients registered with us for home care may avail themselves of our other services such as in-patient and day centre cares should the need arises. The Home Care nurse will assess the situation to ensure continuity of care. The nurse will make the necessary arrangements for In-patient or Day Centre admission.


Cost

We do not charge patients for any of the services provided by Home Care. However, patients are required to purchase their own medication and other consumables such as wound dressings.

We rely on the Ministry of Health’s (MOH) subsidy for your care. We would need to conduct a means testing assessment to check if you are entitled to this subsidy. This can only be done with supporting documents from you and your family. We seek your kind co-operation to provide the required information and document/s for the means test.

Facing the final stage of life

Assisi Hospice is committed to walking this journey with you and your family members. When death becomes imminent, our Assisi team will walk hand-in-hand to give you all the support we can offer. We will provide you and your loved ones all the necessary information and assistance.
